ChatGPT Windows Desktop: Codex shows “Unable to load organization settings” before composer/session, while Web + CLI work #48324

Codex inside the ChatGPT Windows desktop app fails immediately with “Unable to load organization settings” and stops before the Codex composer loads. Because the composer never appears, no desktop Codex session is created and I cannot type / to submit feedback or obtain a desktop session ID.
Environment

  • OS: Windows 11
  • ChatGPT Windows Desktop (MSIX/AppX) version: 26.924.1866.0
  • Codex CLI present on system: 0.158.0-alpha.2
  • Auth: ChatGPT OAuth

Steps to reproduce

  1. Launch ChatGPT Windows desktop app
  2. Sign in successfully
  3. Open Codex
  4. Immediately see dialog: “Unable to load organization settings” (Retry / Sign out / Exit)
  5. Retry repeats indefinitely

Expected
Codex loads and shows the composer.

Actual
Codex stops at organization-settings initialization; composer never renders; no desktop session is created.

Comparison (same account)

  • Web: https://chatgpt.com/codex works normally
  • CLI: logged in via ChatGPT and model requests succeed (returns OK)
  • codex doctor: auth/config/HTTP/WebSocket connectivity all OK (incl. successful WS handshake)

This appears isolated to the Codex integration in the ChatGPT Windows desktop app (desktop-specific org-settings fetch/init path).

Troubleshooting performed

  • Windows app Reset + full reinstall
  • Re-authenticated (fresh %USERPROFILE%.codex\auth.json created)
  • Verified no proxy / WinHTTP proxy
  • Removed stale .codex junction from older migration; removed old switcher config
  • Confirmed no local policy/config files exist:
    • %USERPROFILE%.codex\config.toml (absent)
    • %USERPROFILE%.codex\managed_config.toml (absent)
    • %ProgramData%\OpenAI\Codex\requirements.toml (absent)
  • %USERPROFILE%.codex\sessions contains only CLI sessions (originator: codex_exec); archived_sessions empty (no desktop session is created)

Request
Please advise how to collect Windows desktop app logs/telemetry for this failure mode (when Codex UI fails before session creation), or investigate the desktop org-settings initialization/loading path.
