v0.1.0 - Move main.go to root, TUI visual improvements

BREAKING CHANGE for module path resolution: the package is now at the module root instead of in cmd/ subdirectory.

This means:
- go install github.com/lleontor705/opencode-model-selector@latest now works correctly
- The install path no longer needs /cmd suffix
- Binary name is now correct (opencode-model-selector, not cmd.exe)

Also includes:
- TUI visual improvements: cohesive color palette (Tokyo Night)
- Header banner with project name and version
- Status bar with screen name, model count, dirty indicator
- Better field hints in agent detail screen
- Provider badges in model selection
- Fix isDirty helper in integration tests

Build verified:
- go install . produces opencode-model-selector.exe
- go test -count=1 ./... passes
- golangci-lint run ./... clean
- Binary: 5.8 MB, fully functional
