[BUG] Interrupted · What should Claude do instead? randomly firing across multiple parallel sessions #35741

Description

@rubenbuijs

Preflight Checklist

  • I have searched existing issues and this hasn't been reported yet
  • This is a single bug report (please file separate reports for different bugs)
  • I am using the latest version of Claude Code

What's Wrong?

Mid-task interrupts ("Interrupted · What should Claude do instead?") fire randomly
across all parallel terminal sessions. Restarting Claude Code does not fix it.
This never happened before 2.1.77/2.1.78 with the same setup. Went back to v2.1.76 and it works well again.

What Should Happen?

Parallel Claude Code sessions should run independently without triggering interrupts
in each other. Sessions should resume normally after a restart.

Error Messages/Logs

Steps to Reproduce

  1. Open 2+ terminal windows running Claude Code in parallel on the same project
  2. Start a multi-file editing task in each session
  3. Mid-task, one or more sessions show "Interrupted · What should Claude do instead?"
    without any user input
  4. Typing "continue" resumes briefly but interrupt fires again
  5. Restarting Claude Code does not resolve it — interrupts persist across sessions
  6. Issue resolves on its own after some time (unclear trigger)
