fix(pdf): correct case-sensitive file references in SKILL.md
The actual files are lowercase (forms.md, reference.md), but SKILL.md referenced them in uppercase. This breaks on case-sensitive filesystems (Linux, macOS APFS case-sensitive). Fixes: anthropics#538
